Entrance Credentials
1. FOR INCOMING FRESHMEN
- Form 138 / Report Card / Certificate of ratings for ALS Passers
- Certificate of Good Moral Character
- College Admission Test
- Birth Certificate / NSO Authentical Live Birth
- Additional Requirements per program
- BS Criminology- Police Clearance, Medical Certificate
- BS HRM – Medical Certificate
- Special Requirements for special cases

2. FOR TRANSFEREES
- Honorable Dismissal ( Transfer Credential) from the school
- Transcript of Records
- Certificate of Good Moral Character from the school last attended
